The OneCare Rx team is skilled in providing a seamless transition into a partnership. Our Consultant Pharmacists, Certified Pharmacy Technicians, and the designated Partnership Coordinator orchestrate the entire transition process. They begin with a personal meeting with your health care team to establish communication and answer questions. Relationships with your staff are solidified almost immediately. Prior to the commencement date, the following are implemented:
- Action Plan with timelines submitted to management
- Clinical reviews and records transfer
- In-service educational meetings with staff
- Client support processes and procedures
- Personal introductions to staff, residents and families
It is said that knowledge is power. At OneCare Rx, we believe knowledge is potential power. The transfer of knowledge into practical action is the hallmark of a professional health care team. Our Consultant Pharmacists are members of the American Society of Consultant Pharmacists (ASCP). They are experienced educators committed to expanding the knowledge of your health care team. Their commitment extends to residents’ families as well. Meetings can be arranged with family members to discuss a resident’s specific needs and explain the recommended treatment. Reference materials included as part of the educational process are:
- Clinical Guidelines
- Medication Handbooks
- Policy and Procedure Manuals
Accuracy is assured with unit dose packaging. The OneCare Rx team communicates consistent, detailed operational instructions and follow-up to your staff. Our Medical Administration Records (MAR) supply the documents needed to prevent any potential for errors. Medication management includes:

With years of experience in geriatrics and long-term care, they are proficient in the most current pharmaceutical practices, medication interaction evaluation and regulatory compliance. Consultant Pharmacists review the medications of new residents prior to admission, as well as review an existing resident’s change in medication before a scheduled monthly visit. The OneCare Rx Consultant Pharmacist outlines potential medication side effects and makes recommendations to the medical staff. Optimum care for your residents is our highest priority. The Consultant Pharmacist’s role includes:
- Monitoring medications
- Providing formulary assistance to limit out-of-pocket costs
- Informing staff of alternative medications for those not covered by Medicare D

Compounding combines an ageless art with advanced medical knowledge and the latest technology. Our specially trained professionals prepare customized medications to meet the individual needs of the patient.
OneCare Rx pharmacists are able to compound medications in dosage forms and amounts that are not available commercially. This allows prescribers to customize medications to each patient’s specific needs. Dosage forms include lollipops, transdermals, troches, sublingual sprays, nose drops, injectables, ophthalmics and suppositories. Prescriptions can be compounded to contain multiple active ingredients without the addition of added sugars, dyes, alcohol and preservatives.
We also specialize in Wound Management, Pain Management, Infusion Therapies (Hydration, Antibiotic and TPN) and Hospice Care.
OneCare Rx is leading the way with QS1 pharmacy operating software. This system offers interfacing with any electronic medication administration records software (eMAR) using a standard HL7 programming code. Digital files are accurate and readily transferable electronically to medical professionals and other care providers. Benefits include:
- Medical records customization
- Physicians can ePrescribe from any personal computer
- Electronic delivery status and receipt
OneCare Rx utilizes bar coding technology. Bar code verifications assure safe and efficient med-passes. Some eMAR systems allow for the automatic refilling of orders, the viewing and downloading of a resident’s ancillary orders and the creation of real time reports. OneCare Rx offers solutions for every client. In the absence of an electronic records system, OneCare Rx creates a customized approach to managing your medication records. Customized forms include:
- Medical Administration Records (MAR)
- Physician Order Forms
- Treatment Forms
- Behavior Monitoring Flow Sheets
- Psychotropic Medications Administration Flow Sheets
